Are you interested to put contact page in your blog like I put on my blog [DEMO]
If yes then this is very easy by just using one WordPress plugin Contact Form 7. Just install and activate this plugin. After activating this plugin now create a page named Contact or whatever you want and paste this code in that page
[ contact-form 1 "Contact form 1" ]
This is a default contact page setting. If you want to change settings of this plugin or want to add more contact pages then above code may change. Just follow what setting page says.
Now your blog is ready with a contact page
